Meaning of positive Borrelia-specific IgM

  • Detectable Borrelia-specific IgM remains biologically plausible despite low total IgM; serum concentration alone does not predict whether antigen-specific antibodies can be made.
  • A positive result is not proof of current infection. IgM assays are vulnerable to nonspecific and cross-reactive antibodies from other infections or autoimmune disease, and Borrelia IgM can persist after resolved infection.
  • In clinical cohorts, 53% of reported positive IgM immunoblots were judged false positive, mainly isolated-IgM/negative-IgG findings with symptoms beyond four weeks; another laboratory cohort estimated a 25.5% false-positive rate for isolated IgM immunoblots.

  • Use validated two-tier testing. CDC guidance limits meaningful diagnostic use of IgM to symptoms of ≤30 days; after this, isolated IgM should be disregarded in favor of IgG interpretation.
  • With recent, compatible illness and credible exposure, IgM can be supportive evidence. If suspicion remains in an untreated patient, repeat testing in 7–14 days for IgG seroconversion.
